Terms & Conditions For Training At Women's Self Defense Boston

SCOPE OF TERMS

LIABILITY & RESPONSIBILITY

In consideration of the person submiting this form being allowed to attend Women's Self-Defense Classes, I acknowledge that Self-Defense training is a physical activity and that participation may result in serious injury including death, and or other damage, due not only to their actions, inaction or negligence, but also to that of others associated with and or present at these or other classes.

Furthermore, I acknowledge that there may be risks not known to me or the participant and or not reasonably foreseeable at this time. I personally, totally and completely assume all risks involved or in any way related to their participation in this Self-Defense class whether known or unknown to me at the present time.

Participation in this Self-Defense class is voluntary and entirely of mine and their own free will. I hereby represent and certify that I/they are in good physical condition, and have no diseases, injuries or other physical or mental conditions that would impair their ability to participate in the Self-Defense class. I, my heirs, executors and successors hereby waive and claim of accidental and or negligent tort damages or injury against SEPS Women's Self Defense Program Boston and its instructors. I also certify that I/they are covered by a medical insurance plan and if asked will present the needed information for approval to attend and participate in one of the SEPS Women's Self Defense Program Boston classes.

VIDEO, AUDIO & INTELLECTUAL PROPERTY

Whilst training on the premises of SEPS Women's Self Defense Program, and/or at events and seminars organized or arranged by SEPS Women's Self Defense Program, I acknowledge that photographs or video may be taken, and I give SEPS Women's Self Defense Program permission to use these photographs and video containing the participant's image for any and all purposes. By signing below, I guarantee that in accordance with the rules of SEPS Women's Self Defense Program, they will not make any recording, in any format: video, audio, or photography, within the SEPS Women's Self Defense Program facility. I further understand that by accessing content on the KMY Online site I am agreeing to the Terms & Conditions set forth on the site.

CORPORATE TRAINING

The WOMEN'S SELF-DEFENSE BOSTON program offered by KRAV MAGA YASHIR BOSTON is offered to individual women looking for SELF-DEFENSE & PERSONAL SAFETY TRAINING. It is not offered to commercial enterprises and organizations looking to provide training for their employees and clients. By booking this class/seminar, I acknowledge that I am booking as an individual and not part of a larger group, or organization.

CHILD CARE

By booking this class I acknowledge that The WOMEN'S SELF-DEFENSE BOSTON program offered by KRAV MAGA YASHIR BOSTON does not have a childcare component to it, and that no children/minors can be left unsupervised in the KRAV MAGA YASHIR BOSTON facility.

THIRD PARTIES

By booking this class I acknowledge that The WOMEN'S SELF-DEFENSE BOSTON program offered by KRAV MAGA YASHIR BOSTON does not allow third parties to watch any classes or seminars nor wait in its facility.
